Please refer to the lease entitlement notice or the original lease application form (Form 1) which was used to apply for a lease under the Aborigines and Torres Strait Islanders (Land Holding) Act 1985.
If you have a lease entitlement for which a notice has not been published please provide the evidence listed below so that it can be checked to see if an additional notice should be published.
Complete this section if the details published about a lease entitlement are incorrect and a replacement notice should be published. Indicate the changes required and attach supporting documents.
Complete this section if you want to surrender all or part of the lease entitlement.
Complete this section if you want to amend the statement of reasons (obstacles) to say that no practical obstacles exist to the grant of the lease.
